| Job Duties |
1. Collaborates with other disciplines, physicians and payors to
assess, develop, implement and monitor comprehensive plan of care
to ensure continuity of care and cost effectiveness.
2. Collaborates with physicians and staff regarding medical necessity
for admission, continued stay or alternative treatment facilities
and/or levels of care. Collaborates with Social Work and others to
arrange discharge planning referrals per physician orders.
3. Relates utilization assessment to payors that are needed to justify
continued stay review and to obtain pre-certification.
4. Assesses appropriateness of level of care on concurrent basis.
5. Evaluates effectiveness of care and facilitates coordination of
care toward specific outcomes criteria.
6. Maintains timely documentation. Meets department and St. Paul
University Hospital documentation standards.
7. Identifies quality and risk issues, makes appropriate referrals.
Confers with supervisor and/or Medical Director as appropriate.
8. Communicates effectively with other health care providers,
patients, families, other hospital personnel and visitors.
Demonstrates effective communication with adolescents, adults and
older adults.
9. Demonstrates knowledge of unit performance improvement and is aware
of expected participation.
10. Identifies need for professional growth and seeks appropriate
development opportunities.
11. Evaluates care management outcomes data to identify trends and
areas for improvement.
12. Performs other duties as assigned.
